Utilizing our carrier’s Preferred Provider network, employees have access to providers and facilities nationwide. All preventive screenings received by network providers are 100% covered.
You may choose from three comprehensive dental plans. Each dental plan includes coverage for a spouse or domestic partner and/or dependent children. All of our dental plans cover preventive and basic restorative services. You may also choose a plan which includes coverage for major restorative and orthodontic services. And you have the flexibility to choose any dental provider, or use the Preferred Dentist Program provider to lower your out-of-pocket costs.
Our vision plan gives you access to a national network of optometrists, ophthalmologists, and opticians for your eye care needs. Your vision benefit provides savings up to 40% on your eyewear needs.
Post-employment health care savings account
The PHSA is allows you to save for post-employment health care expenses via current payroll deductions. Any earnings realized in your account are tax-free. After your employment terminates, the money in your account is not taxed when you or a dependent request reimbursement for an eligible health care expense.
Disability and life plans
In addition to employer-paid short- and long-term disability insurance, you also have options of life insurance and optional accidental death and dismemberment insurance for yourself, spouse, domestic partner, and/or children.
